How do I register for dental services with you?
Simply complete the online registration form above and we’ll come back. to you as soon as we can to progress your registration and get your initial appointment booked in.
What documentation do I need to register?
We’ll need some documentation from you to complete the registration progress such as proof of address, medical history information and contact details. We’ll confirm all documentation required though as you progress through the registration process.
When will I be required to make a payment?
We may ask you to pay a deposit to book certain services with us in advance, but this will all be discussed and explained during your consultation call with one of our friendly team and this is your chance to ask any questions that you have.
